Here are the on-line documents for the Valentine First Day Packet.  They include the four forms that must be filled out, signed, dated, and returned to the school in the envelope your child will bring home the first day of school. 

Technical Tip:  You must use Adobe Acrobat Reader or any Adobe Acrobat version to complete the fill-able forms.  Attention Apple Mac users:  PDF documents may open in "Preview" and will not be fill-able.  Please download the forms and open the form using Adobe Acrobat Reader.  If you need assistance setting your MAC to open using Adobe Acrobat, click here.

Please view the tutorial for each form before you go to the PDF form.  The tutorial will take you through the process of filling out each form; it also contains valuable information.

Once you have filled out and printed each form, you will need to sign, date, and fill in the teacher’s name on each form before you return them to school.  Your student will bring home an envelope so that you can return the forms to school.

Tutorials are auditory as well as visual.  Please turn your computer speakers on.  The Flash player is required to view the tutorials.  If you do not have Flash, you can download it for free here.  These tutorial files are large and may take a few minutes to load.   Once the tutorial is loaded, click the play button to start.
